Publication of the 2nd issue in 2019

Prof. Lieping Chen et al. published their work on immune suppressor Siglec-15 in Nature Medicine at the beginning of this year. According to this breakthrough research, Prof. Xiubao Ren accepted our invitation and contributed an editorial with perspective on a therapeutic application related to this new target. In this latest issue, you could appreciate Prof. Ren’s work and learn about his viewpoints.

Supporting by a large population study, Prof. Martha L. Slattery et al. composed a review which discusses the functional role of miRNAs and colorectal cancer. As for the results of original researches, including the molecular mechanisms in tumorigenesis or cancer development associated with signal pathways, the application of new drugs in cancer treatment, and so on, should be also useful and worthy of reading by oncologic clinicians and researchers.

We hope all of these articles will greatly benefit your research and clinical practice. Thank you for your attention and welcome to submit your work to Cancer Biology & Medicine.

Publication of the 1st issue in 2019

The 1st issue in 2019 has been published. An editorial and a Chinese expert consensus offer the advances and insights in clinical application of next-generation sequencing. Another editorial reports a breakthrough in in vitro expansion of human primary hepatocytes. Review of Jianguo Chen et al. shows the epidemiological trend and risk factors of liver cancer in Qidong, an area with high incidence. What’s more, two original researches from China are worthy of recommending. One study on reactive capillary hemangiomas from Xuelian Chen et al. could arouse an awareness of drug toxicities in immunotherapy. Another one written by Xiaohong Wu et al. reports that BRAF mutation and TIRADS classification could be as tools in diagnosis of papillary thyroid cancer. Besides, several latest results from basic and clinical researches were also presented, especially studies on signaling pathways. We hope all of these articles will greatly benefit your research and clinical practice.

Special Issue on Geriatric Oncology

The special issue on geriatric oncology has been co-published by Chinese Anti-Cancer Association (CACA) and International Society of Geriatric Oncology (SIOG), the letters from editor-in-chief and guest editor are as followed:

Letter from the Editor-in-Chief

Dear audience,

With the aging of population worldwide, we are faced with an increasing burden of cancer in the elderly. GLOBOCAN 2012 reported that there are 14.07 million new cancer cases worldwide every year, with 8.45 million patients older than 60 years. Of 8.2 million cancer deaths each year, those of patients older than 60 years are 5.7 million. According to Chinese Cancer Registry Annual Report 2014, in 2012 those aged >60 years accounted for 57.93% of all cancer patients; and cancer deaths of this age group patients accounted for 70.7%.

In order to improve the survival and quality of life of geriatric cancer patients, oncologists should perform a thorough evaluation and make individualized treatment plans. In January 2015, the society of geriatric oncology in the Chinese Anti-Cancer Association was established. Chinese oncologists are making joint effort in promoting geriatric cancer research. But they are still challenged in assessing and treating elderly cancer patients.

The 2015 ASCO Annual Meeting highlighted the needs of geriatric cancer patients and corresponding critical issues in treatment, increasing attention of cancer researchers and clinicians to geriatric oncology. Cooperated with the International Society of Geriatric Oncology (SIOG), Cancer Biology & Medicine invited Professor Riccardo A. Audisio, the past president of SIOG and a world famous expert in geriatric oncology, to organize this special issue. The contributing authors have outstanding expertise in geriatric oncology and the contents involve assessment of fitness for surgery, chemotherapy tolerance and clinical trials in the elderly cancer patient.

I sincerely expect these excellent articles will be helpful to your research and clinical practice. I am grateful to the hard work of Professor Riccardo A. Audisio, Ms. Laurence Verhagen (chief executive officer of SIOG), and all of the authors.

At last, on behalf of the editorial team of Cancer Biology & Medicine, I wish you a happy and fruitful new year of 2016.

Letter from the Guest Editor

Dear colleague,

Geriatric Oncology is not a specialty, it's an attitude. There is nothing special in treating senior patients affected by cancer; we all do that in our everyday clinical practice. The problem rests when you want to deliver the best possible treatment to each individual older patient.

This needs some unique understanding of the complexity of aging: let's start saying that one in two older patients suffers from malnutrition, lack of mobility, incontinence and cognitive impairment. And let's also say that one in four suffers from major psychosocial issues. This is no normal individual. Are we sure that we entirely appreciate this complexity when deciding how best to treat older patients with a malignant tumor?

The statistics show how bad we are in treating this patients' group; we can share responsibilities and therapeutic failures however. Since surgery is the main tool to defeat cancer, the poor cancer-related survival is predominantly a surgical failure.

And why is that? We do over-treat older patients, when we are incapable of entirely appreciating their vulnerability and modest life expectancy. And yes, we do under-treat these patients as well, predominantly when we are inappropriately concerned with their unproven frailty.

So the key issue is frailty assessment: checking for malnutrition, depression and functional inability is mandatory. Frailty assessment is not only possible, it is rather easy and very cheap. It should be implemented into our daily practice. Screening tools have been tested and are being more and more utilized. They are pivotal in deciding the optimal management of older cancer patients. Is this rocket science? No, it is not: similarly to the TNM classification which was first introduced in the 50's, now at its 7th edition, the tool is not perfect. But we use the TNM system regularly. We depend on it in our decision-making process, when presenting our data to the scientific community or comparing results with colleagues. Several screening tools are available and should be widely spread and compared if we want to optimize the way we treat this patients' group.

The cover picture by Michael Howard, an impressive freelance illustrator based in London UK, summarizes the complexity of the issue: the intensity of the decision making process, the vulnerability of older patients, notes of depression as well as the unexpected resources presented by older patients. This delicate drawing elegantly portraits the complexity of the problem and forces us to do better when designing a treatment plan for senior cancer patients.
